The Assessment Process: 3 Simple Steps

Step 1: Intake Appointment (1 Hour)
The goal of this appointment is to get a full picture of your mental health history and current needs. During this initial appointment, you will complete a series of necessary questionnaires and assessments to help us gather a comprehensive understanding of your mental health needs. Our practitioner will take time to review your history, current symptoms, and concerns, as well as discuss any previous treatments or diagnoses. This process allows us to establish a clear baseline of your mental and emotional well-being. The intake is designed to be thorough and supportive, ensuring that we tailor our approach to meet your unique needs moving forward.
Step 2: Cognitive Assessment (35-45 Minutes)
This step helps us gather cognitive insights that are essential for forming an accurate diagnosis. In this step, you will complete a cognitive assessment. The assessment can be done conveniently at home. This tool is designed to evaluate various aspects of cognitive function, providing key insights into areas such as attention, memory, and processing speed. The results from the assessment will guide our practitioner in making an informed diagnosis, ensuring that the next steps in your care are both accurate and personalized to your specific needs.
Step 3: Follow-Up Appointment (30 Minutes)
Here we review your results and work together to create a plan. In the final step, you will meet with your practitioner to discuss the results of the assessment. If a diagnosis has been made, the practitioner will explain it in detail, helping you understand what it means and how it may be affecting your daily life. This session is also an opportunity for you and the practitioner to work together to develop a personalized treatment plan, tailored to address your unique needs and goals. The plan may include recommendations for further evaluations, medication management, or other therapeutic options to support your mental health and well-being moving forward.
